Job Description Advanced Practice Provider (NP or PA) Palliative Care Newark, NJ – Fulltime/Permanent DESCRIPTION Advanced Practice Provider (NP or PA) In-Person Role | Hospital & Post-Acute Settings | NJ License Required Seeking a compassionate and skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing team. This is an In-person position serving patients in Essex, Hudson, Union, Middlesex, Somerset, Monmouth, and Ocean counties. Responsibilities: Deliver comprehensive palliative care in hospital and post-acute care environments Conduct patient assessments, develop care plans, and manage ongoing treatment Provide education and support to patients and families regarding goals of care Collaborate with physicians, facility staff, and community partners to ensure continuity of care Document clinical encounters in compliance with regulatory and organizational standards Qualifications: Active and unrestricted New Jersey Nurse Practitioner (NP) or Physician Assistant (PA) license (required) Experience in palliative care, geriatrics, internal medicine, or related field preferred Strong communication, interpersonal, and critical thinking skills Commitment to patient-centered, compassionate care Compensation & Growth Opportunities: Competitive pay with bonus potential Candidates with 10–15 years of dedicated palliative care experience may qualify for compensation at the higher end of the pay range Team Lead roles are available for highly experienced APPs, offering enhanced responsibilities and compensation Incentives & Benefits: Up to 12.5% bonus based on quality and productivity metrics Comprehensive medical, dental, and vision coverage Malpractice and professional liability insurance provided Long-term disability coverage Annual CME allowance Paid DEA & license renewals Tuition reimbursement Additional benefits including pet insurance, life insurance, and more.
