Position TravelNurseSource is working with Cynet Health to find a
qualified Home Health RN in Bronx, New York, 10451! Job Title:
Registered Nurse (RN) Profession: Nursing Specialty: Home Health
Duration: 260 days Shift: Day Hours per Shift: 8 Experience: Two
(2) years of experience in a home health setting within the last
fifteen (15) years License: Current RN Licensure in New York State
Certifications: BLS required before the start date Must-Have:
Recent experience conducting comprehensive assessments of patients
Ability to review health status questions and make evaluations
based on criteria for enrollment Knowledge of the theory,
principles, and practices of general professional nursing Excellent
organizational, interpersonal, written, and verbal communication
skills Ability to work effectively with multi-generational
consumers Ability to perform comfortably in a fast-paced,
deadline-oriented work environment Strong ability to work
independently Description: Perform initial in-home pre-evaluation
of patients that includes evaluation of health status, strengths,
care needs, and preferences. Guide the development of
individualized long-term care service plans. Review consumer
medical documentation and health referral forms relevant to the
case. Enter evaluation data into electronic forms and transmit as
required. Document concerns, conflicting information, and other
issues that arise during the evaluation process. Emphasize
continuity of care to reduce or eliminate fragmentation,
duplication, and gaps in treatment plans. Discuss healthcare
options, supports needed, service vendor options, and waiver
options with consumers. Initiate communication between the call
center and eligible consumers for enrollment into managed care
health plans. Enhance communication and collaborative relationships
with interdisciplinary care team members to improve care
coordination and facilitate service delivery. Collect quality
review data and any required documentation to support outcome
measurements. Record case notes into the CRM system. Identify
opportunities for health promotion and illness prevention. Maintain
a comprehensive working knowledge of community resources and
network services for the target population. Meet all standards
established for the position as outlined in the corresponding
annual performance criteria. Perform other duties as assigned by
