Registered Nurse (RN) | Home Health Location: Bristol, CT Agency: Cynet Health Pay: $2,424 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Home Health/Hospice Registered Nurse Profession: Registered Nurse Specialty: Home Health/Hospice Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: 1 year of home care or hospice experience License: Valid RN license required Certifications: BLS certification, valid driver's license, and insurance required Must-Have: Experience with Meditech home care EMR is preferred Description: The position involves providing home health and hospice care to adult and geriatric patients. A reliable vehicle, valid driver's license, and auto insurance are required for this position. The RN will see an average of 5-6 patients daily based on a point system for visits. Common diagnoses include medication management, COPD, post-surgical care, and dementia. Evaluations and development of a plan of care are essential duties. Weekend call is required, and the RN must respond to client calls within 20 minutes and visit within 30 minutes on weekends. Self-scheduling is permitted within the outlined shift hours, with rotation for weekend coverage. Professional attire is expected while on duty.
